We left out performance claims we couldn't verify. When we say an agency may be the wrong fit, that describes who it is built for. It isn't a judgment of the work.

What an orthodontic marketing agency should improve
Before comparing agencies, work out which number you want to move. Orthodontic growth happens in three stages, and most agencies are strong in one or two.
- Getting found: parents searching for braces and adults searching for Invisalign, on Google, in Maps and more and more in ChatGPT and Gemini. Websites, local SEO, reviews and ads live here.
- Getting booked: the inquiry becoming an NP exam. That comes down to how fast your team calls a web inquiry back, whether a 7pm call reaches anyone, and how easy the site makes booking.
- Getting started: the exam becoming a contract. Your TC and your financing matter most here, along with whether anyone calls back the families who said they needed to think about it.
An agency that only works on the first stage can double your traffic and leave starts flat. If inquiries are already coming in, the second and third stages are where the money is.
How to choose: questions to ask on the first call
- How will you report results? Ask to see a real report from a current client. It should count consults booked and starts, because impressions and clicks don't pay the bills.
- Who owns the accounts? Your domain, website, Google Business Profile, ad accounts and call tracking numbers should be in the practice's name.
- How long is the contract, and what happens if it isn't working? Six to twelve months is common. Month to month, or a performance guarantee, puts the risk on the agency.
- Do you work with another orthodontist near me? Plenty of agencies take competing practices in the same town, so ask directly.
- What happens to a web inquiry at 7pm? If the answer is that the front desk calls tomorrow, the practice that calls back tonight will probably book that family.
- What do you need from my team? An agency that measures starts will want your practice software reports and a monthly check-in with your TC. One that needs nothing from you isn't counting starts.
What orthodontic marketing costs
Few orthodontic agencies publish prices, which makes comparison harder than it should be. What you will usually see is a monthly management fee plus a separate ad budget paid to Google and Meta. Keep the two apart when you compare quotes. An agency quoting one combined figure may be spending less on ads than you think.
A better comparison is cost per start. Take everything you spend on marketing in a month, including the agency fee, and divide by the starts it produced. An agency that can't help you calculate that number can't show you it is going down.
